Registered Nurse - Winchester/Warren/Clarke counties - 8 Hr shifts

Description:The Registered Nurse is responsible for directing the total health care services for patients from admission to discharge. He/she provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of this position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness. The Registered Nurse acts as a liaison between patients, family members and other health care personnel to ensure efficient and appropriate home health care services.
Position is responsible to an in-house RN clinical supervisor.
Duties:Qualifications:

1. Current license to practice nursing issued by the state of employment.

2. Successful completion of State-accredited training program in Bachelor of Science in Nursing or Associate Degree in Nursing.

3. Successful passing of the National Council Licensure Examination – Registered Nursing.

4. Minimum of two (2) years experience in home health, acute care or rehabilitation setting, preferred.

5. Current CPR certification.

6. Must be able to direct care appropriately to support the specific populations (adult & geriatric) provided in nursing services.

7. Experience in office procedures:

a. Ability to work independently with minimal supervision.
b. Ability to resolve problems and set priorities.
c. Excellent written and verbal skills to effectively communicate with confidence, clarity and attention to detail.
d. Pleasant telephone demeanor with exceptional customer service skills.
e. Basic typing and computer skills.
f. Ability to work cohesively with others and multi-task in a busy office setting.

8. Satisfactory references.

9. TB Test or Chest X-Ray 6 months prior to employment or within 30 days of employment.

10. Successful completion of orientation process.

Job Knowledge:

1. Possess working knowledge and understanding of the functional organizational make-up of the Home Health Agency.

2. Ability to demonstrate, and provide examples of, continuous personal development and improvement in each of the essential functions listed in the section below:

• Ability to make independent decisions after gathering information from internal and external sources
• A customer-focused attitude

4. Working knowledge of basic computer programs and HealthWyse

5. Excel and Word preferred although not required.

6. Strong background in Home Health, Medical/surgical or Rehabilitation setting preferred.

7. Infusion knowledge, skills and experience are considered a benefit for this position.

Responsibilities:

1. Act as a comprehensive care coordinator for patients assigned to his/her care, assessing the patient’s needs accordingly and facilitates communication amongst other clinicians providing care and the clinical supervisor.

2. Demonstrate appropriate judgment skills to be able to make independent clinical decisions in patient care matters.

3. Promote the level of clinical expertise required of staff to provide safe, exceptional and effective nursing care.

4. Establish an appropriate plan of care which addresses the patient’s functional problems.

5. Identify appropriate short and long-term goals for each patient.

6. Reviews results of laboratory tests and other diagnostic procedures and reports to the physician appropriately.

7. Supervise License Practical Nurses (LPNs), Certified Home Health Aides (CNAs), Medical Technicians and Medical Assistants according to Federal, State, and Agency policies.

8. Perform documentation according to the Agency’s policies and procedures.

9. Maintain regular communication with members of the interdisciplinary team regarding progress of the patient, changes in status, and discharge from services.

10. Maintain communication with the physician regarding status of the patient including: initiation of treatment plan, changes in status, and discharge from care.

11. Participate in patient care conferences as needed.

12. Attends appropriate in-service programs to keep current in field.

13. Complete monthly in-services.

14. Performs other duties as assigned.
